How do I receive the items I win?

Items can be picked up at the Lake Health Administration Building in Concord Township on the following dates with proper photo identification:

· Wednesday, Nov. 18th 10:00am-5:00pm

· Thursday, Nov. 19th 10:00am-5:00pm

· Friday, Nov. 20th 10:00am-5:00pm

If you need other arrangements, please work with our Lake Health Foundation Office by emailing [email protected] or calling 440-354-1934. You won't even have to get out of your car!

All my items are gift cards/certificates. Do I still need to pick them up?

If all of the items you won are gift cards or gift certificates, we would be happy to mail them to you at the address provided on your profile by certified mail. However, Lake Health Foundation is not responsible for items not received.

I was the lucky winner of the 50/50 Raffle, how will I receive the funds?

The winner of the 50/50 raffle will be required to complete the proper tax form and return it to Lake Health Foundation. Once received, we will request a check and mail it the address provided by the winner on the tax form.

Can I buy packages that include alcohol if I am under 21 years of age?

You must be 21 years of age or older to purchase items where alcohol is included.

If I am not looking to purchase gifts right now but still want to support Lake Health Foundation, what are my options?
